I think this is the most adorable pincushion I've seen.  Jen of verybigjen has a complete pictorial tutorial on Flickr.  All of her pincushions, and there are several, all begin with a bottle cap.  Several of her pincushions are featured as projects in the book “Pretty little Pincushions” from Lark Books available on Amazon and have twenty-one awesome  4 and 5 star reviews.
